Register on QForm and add your space
Create a form and give it a name. It is only visible in QForm
In the designer mode, place the fields "Name", "Phone". Configure them: specify the headings, text inside the field and make them mandatory to fill in
Drag the Hidden Field type by specifying the field class in the For Developers panel

Go to the "Embed code" tab

Copy the DIV to install the form and the script to call it, paste it on the site

After installing the code, the created feedback form will appear on the site. The hidden field will not be displayed on the page with the form.
